Accepting New Patients

Your First Visit to Rancho Oral and Facial Surgery & Dental Implant Center

Your first visit will include a pre-surgical evaluation with a doctor about your diagnosis and options for treatment. After a thorough review of your medical history and examination of your condition, an appointment will be scheduled for your surgical procedure. 

At least 3 days prior to your appointment please fully complete the online registration and medical history.  Please make sure to complete the medical and dental insurance information so we can give you an estimate of your out-of-pocket expenses for your visit. If you do not have this information or do not provide this information for us prior to your visit you will be responsible for the full amount of your visit.  We will provide you insurance ready receipt that you will be able to submit to your insurance company for reimbursement.  The following information is important to bring to your pre-surgical evaluation appointment if you did not included it in your online registration:

  • Any x-rays and surgical referral note, if applicable
  • A list of your current medications
  • Completed forms for medical or dental insurance

IMPORTANT: A parent or guardian must accompany a patient under the age of 18 at the consultation visit.

Some medical conditions and medications may be a concern prior to surgery and it is important for you to notify the office in advance. If you have a medical condition such as diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc., or are taking medication such as aspirin, anticoagulant therapy, heart medications, etc., please alert us. If your x-rays have been taken at your dentist or physician’s office, it will be helpful if you request that they forward them to our office. In the event that there isn’t enough time for the x-rays to be forwarded, please get the x-rays directly from your dentist or physician and bring them to our office. If we do not have x-rays to evaluate your condition, additional x-rays will need to be taken at our facility and will be your responsibility for the cost of the x-rays.
